Preparing for your first appointment

  • To enable time to complete your paperwork, arrive 15 minutes early for your first visit.
  • Do you have any questions? Make a list of questions you’d like to ask during your first meeting.
  • Bring all of your documents with you. This includes a photo ID, a copy of your insurance card and the script from the doctor if you have one.

  • Please call our office ahead of time to verify your insurance benefits at 860-413-9969.
  • Prepare to talk about your medical history, previous treatments, current medications, and other relevant details.
  • Make a list of any symptoms or signals the therapist should be aware of. What has recently irritated or wounded you? How long have you been working on this problem? What are you unable to perform as a result of it?

What to expect at Connecticut Physical Therapy Specialists during your first appointment:

Arriving at our physical therapy clinic is similar to arriving at any medical office! Here is what you can expect.

  • 24 hours before your appointment you will receive a text message or phone call reminding you of your appointment.
  • For patients who choose to receive a text message for their reminder, you will receive a separate text with a link. When you click on the link you will be able to answer questions related to your health history and a quick questionnaire about the reason we are seeing you. For any new patient, there is always the option of completing this same health history in paper form at the office.
  • You will register at the front desk when you arrive at our office. We’ll ask for your insurance information and any other paperwork you may have brought with you (i.e., referral, X-ray or MRI report, other medical reports)
  • After that, you’ll have your initial evaluation with one of our doctors of physical therapy. Our therapists conduct a thorough assessment, including your past medical history and your current pain, illness, or injury. This is essential to helping you, so please be open and honest with us. We genuinely want to listen and understand your problem(s) and provide you with the best possible solution!
  • Our physical therapist will conduct a comprehensive physical examination to evaluate your range of motion, strength, and movement patterns during your appointment.
  • Based on your evaluation findings, our doctors of physical therapy will devise a therapy plan for you. On the first day, you will likely receive an explanation, education, and some home exercises to do. Each session after that will build on the initial evaluation to create the most significant impact possible for each therapy session. The goal will be to help you recover as quickly as possible and achieve meaningful, lasting results.
  • Our therapists will record their findings and treatment plan, and they can give those to any medical expert of your choosing.
  • At your first session, our therapist will discuss your goals and, based on sound reasoning and clinical experience, determine how long and how frequently you should attend your physical therapy sessions.
  • You will arrange the remaining sessions at the front desk before leaving your initial appointment. We will check your insurance to see if you have a cost share and will let you know the payment amount on your second visit.
  • You may contact us at any time if you have any questions. We want to make sure you are comfortable as you get back to normal as soon as possible.

If you wish to save time on your first visit, please feel free to print the patient forms below, fill them out, and bring them with you to your appointment. If you do not have time to print them ahead of time, arrive at least 15 minutes early to give yourself plenty of time to finish them before your appointment.
